These Terms & Conditions explain the rules for using the Cumberland Health mobile app and related services (“Cumberland Health,” “we,” “us,” or “our”). Please read them carefully.

By creating an account or using the Cumberland Health app, you agree to follow these Terms & Conditions. If you do not agree, do not use the app.


1. About Cumberland Health

Cumberland Health is a telehealth app operated by Cumberland Health, a subsidiary of Cumberland Family Medical Center, Inc. (CFMC), a Federally Qualified Health Center (FQHC) based in Kentucky.

All medical services provided through the Cumberland Health app are delivered by licensed medical providers who are employees of CFMC and licensed to practice in the state of Kentucky.


2. Who May Use the App

  • Cumberland Health is available only to patients physically located in Kentucky at the time of service.
  • Adults may create and manage their own accounts.
  • Minors may receive care only through a parent or legal guardian–managed profile.
  • The parent or legal guardian is responsible for providing accurate information, giving consent, and making payments for dependent profiles.

3. Medical Services and Limitations

Cumberland Health provides access to non-emergency medical services, which may include primary care, general medical care, pediatrics, women’s health, behavioral health, nutrition, physical therapy, and chronic condition care.

Not for Emergencies

Cumberland Health is not for medical emergencies.

  • If you have a medical emergency, call 911.
  • If you are experiencing emotional distress or thoughts of self-harm, call or text 988, the Suicide & Crisis Lifeline.

4. Provider Judgment and Prescriptions

All care decisions are made by licensed medical providers using their professional judgment.

  • Providers may prescribe medications and authorize refills when medically appropriate.
  • Prescribing is subject to state and federal law.
  • Some conditions, treatments, or medications may require an in-person visit.
  • Cumberland Health does not guarantee any diagnosis, treatment, prescription, or refill.

5. Your Responsibilities

You agree to:

  • Provide true, complete, and accurate information
  • Keep your account login information secure
  • Use the app only for lawful and appropriate purposes
  • Treat providers and staff with respect

You are responsible for all activity that occurs under your account.


6. Prohibited Use

You may not:

  • Use the app for emergencies
  • Share your account with others
  • Provide false or misleading information
  • Harass, threaten, or abuse providers or staff
  • Attempt to interfere with the app’s security or operation

We may limit, suspend, or end access to the app if these rules are violated.


7. Billing and Payments

Cumberland Health visits are self-pay.

  • The standard self-pay fee is $40 per visit
  • There are no subscriptions or recurring fees

Cumberland Family Medical Center, Inc.:

  • Accepts Medicare
  • Accepts Medicaid
  • Accepts Sliding Fee Program if it is already applied to patients

Sliding fee discounts are not available through the Cumberland Health app and require a clinic visit and proof of income.


8. No Insurance Guarantee

Using Cumberland Health does not guarantee insurance coverage or payment by any insurance plan. You are responsible for all charges associated with app-based visits.


9. Privacy

Your health information is protected by law, including HIPAA.

  • We use your information to provide care and operate the app
  • We do not sell personal or health information
  • Details about how we protect your information are explained in our Privacy Policy

10. Communications

By using the app, you agree that Cumberland Health may contact you through the following methods:

  • In-app messages
  • Push notifications
  • Text messages
  • Email

Certain notifications related to your care and visits are required so that we can safely provide services through the app. These may include appointment confirmations, visit reminders, provider messages, and important account updates.

You can choose how you would like to receive optional notifications, such as general announcements or promotional messages, by adjusting your communication preferences in the app.

You may stop receiving optional text messages at any time by replying STOP. Push notifications can also be turned on or off through your device settings or your notification preferences within the app.


11. Technology and Security

You are responsible for:

  • Having a compatible device
  • Maintaining internet access
  • Protecting your device and login credentials

We work to keep the app secure, but no system is completely risk-free.


12. Changes to These Terms

We may update these Terms & Conditions from time to time. Continued use of the app means you accept the updated terms.


13. Governing Law

These Terms & Conditions are governed by the laws of the Commonwealth of Kentucky. Any legal action related to these terms must be brought in Kentucky courts.
